Attestation sheets that survive a second reader
Signer names, thresholds, and wallet IDs — the three fields that most often drift between ceremony and custody report.
Attestation packs fail reconciliation when the ceremony form and the custody report use different wallet nicknames. Before the close, print one mapping table: ceremony ID, report line, device serial.
Thresholds should appear as numbers and as prose (“2 of 3”) on the same page. Signer names should match employment or mandate records your auditor already holds — nicknames alone create query loops.
